Client Care Coordinator - LMSW/LMHC

Position Overview
The Client Care Coordinator is responsible for providing comprehensive support to clients, ensuring their needs are met through effective case management and advocacy. This role focuses on promoting mental and behavioral health by coordinating services, facilitating communication, and fostering a supportive environment for clients and their families.

Key Responsibilities

  • Conduct initial assessments and develop treatment plans tailored to individual client needs.
  • Coordinate and monitor client care, ensuring appropriate services are in place and delivered effectively.
  • Advocate for clients, helping them navigate the healthcare system and access necessary resources.
  • Provide crisis intervention and support, assessing situations and implementing appropriate responses.
  • Maintain accurate and timely documentation of client interactions and treatment progress.
  • Collaborate with multidisciplinary teams to enhance client care and improve service delivery.
  • Educate clients and families about mental health conditions and available treatment options.
  • Foster strong interpersonal relationships with clients, demonstrating empathy and active listening skills.
Qualifications
  • Master's Degree in Social Work (LMSW) or Mental Health Counseling (LMHC) required.
  • Experience in counseling, social work, or behavioral health services preferred.
  • Strong understanding of mental health issues and treatment modalities.
  • Proficiency in crisis intervention and case management techniques.
  • Excellent communication and interpersonal skills, with the ability to build rapport with clients and families.
  • Demonstrated ability to work collaboratively within a team environment.
  • Strong documentation skills and attention to detail in treatment planning.
  • Ability to maintain confidentiality and adhere to ethical standards in client care.
